What Is a Wisdom Tooth Extraction?

The third set of molars that erupts in teens and young adults can cause myriad problems if there is not enough space in the mouth to accommodate them. Known as wisdom teeth, these molars are prone to causing problems ranging from becoming stuck underneath the gums (impaction) to infection and causing problems with teeth alignment and bite. To prevent these and other problems, most people will have their wisdom teeth extracted by an experienced oral surgeon.

Wisdom teeth that have fully erupted, but are crowding the mouth will be removed, without the need for an incision. Removing partially or fully impacted wisdom teeth will require an incision in the gums to access the tooth and its root. In some cases, the tooth may need to be broken into smaller portions for complete removal.

Some patients may find that they are good candidates for a coronectomy instead of complete extraction. In a coronectomy, only the visible portion of the tooth is removed, leaving the tooth roots intact.

Patients only need to have their wisdom teeth removed if they are causing problems. Patients who have sufficient room in the mouth to accommodate the third molars will retain their wisdom teeth.  Extractions are only needed if:

What are the signs that wisdom teeth are coming in?
Signs patients experience include sore gums, bleeding gums, jaw stiffness, sensitivity in the back of the mouth, sinus problems, headaches, difficulty chewing, tooth decay, and infections.
How long is recovery from wisdom teeth removal?
Recovery depends on how many teeth were removed, and whether the wisdom teeth were impacted. On average, it takes 7-10 days for swelling to completely subside.

What can I eat after having my wisdom teeth removed?
For the first few days after your surgery, you should stick with soft foods. Scrambled eggs, smoothies, yogurt, soup, and other soft foods are best. Avoid spicy, hard, and crunchy foods for the first 48 hours, and chew slowly and carefully as you introduce harder foods.
Will I be awake while my wisdom teeth are being taken out?
If you have impacted wisdom teeth, you will likely be under general anesthesia. Removal of wisdom teeth that have already broken through the gum line may be performed under local anesthetic or I.V. anesthesia. With local and I.V. anesthesia you remain conscious.
